Follow-up Case Management
As families leave our programs, UMOM New Day Centers' Case Managers continue to work with them for as long as one year to assure that they stay in housing. Our staff assist them in accessing needed services and dealing with various challenges they experience in their first months in affordable housing.

Our SRO clients receive a secure, safe environment, three daily meals, initial case management assessment followed by ongoing monitoring and referrals for other services as needed.

Casa Nueva
One of the greatest challenges families preparing to leave UMOM New Day Centers face is the shortage of permanent affordable housing. The Casa Nueva apartment complex is our first endeavor in answering this desperate call.

In collaboration with Community Services of Arizona, UMOM New Day Centers opened Casa Nueva in spring 2002. This project turned an inner city, run-down, littered vacant lot into a magnificent 64-unit complex consisting of 1-, 2- and 3-bedroom apartments. Twenty-six of the units are designated as affordable housing, targeting families with incomes 30, 40 and 50% of the area’s median.

Casa Nueva is a natural “next step” in the continuum for families completing our Emergency Shelter or Transitional Housing program. At the point that they are ready for the move to affordable housing such as Casa Nueva, they will have developed the skills and confidence they need in order to be self-sufficient.
